Add a Waiver to a Form

Attach waivers directly to your forms to automate compliance tracking and collect user consent during registration.

Adding a waiver to a form ensures users formally review and sign required legal agreements prior to submission. Attaching waivers directly to your forms automates compliance tracking, captures verifiable user consent, and seamlessly integrates liability coverage into your standard registration processes.

💡Tip: A waiver must be created before it can be added to a form. See the Create and Update a Waiver article for more information.

  1. Click Forms > List.

  2. Click the More Actions menu on the form you wish to update.

  3. Click Update. The Form Builder appears:

  4. Click the arrow next to the Waiver field in the Available Fields section.

    The Waiver window appears:

  5. Enter a Label for the field on the form.

  6. Click the Waiver dropdown menu to select the waiver you wish to attach to the form.

  7. (Optional) Enter any other wording desired to replace the text next to the waiver checkbox as shown below.

  8. Click the Update button.

  9. Click the Save button.

💡Tip: To preview how the Terms and Conditions appear to a user, click the Actions button > Preview Existing Form.
