Forms allow you to collect essential information, tracking profiles, and compliance data from your workforce. When creating or updating a form, you can fully customise its layout by adding, removing, configuring, or reordering fields to capture the exact details you need. If you are updating multiple events simultaneously, you can also use the Bulk Update action to apply these form changes across your selections all at once.

Create a Form
Click Forms > List.
Click the Create button. The list of forms appear:
Click to select a form. For a comprehensive list of all forms see the Understanding Form Types article.
Click the Next button. The Form Builder appears:
💡Tip: Click the Save button frequently while building your form to ensure none of your progress is lost.
Add Fields to a Form
Fields can be added to a form via two different methods:
Drag and Drop: Click and hold a field to drag it into the form.
Select Checkbox: Check the box next to a field > Click the Add to form button or the arrow at the end of the field to insert it on the form.
💡Tip: Use the Select Checkbox method to add multiple fields at a time to a form.
💡Tip: Use the search bar to search for fields.
💡Tip: Once you add a custom field to a form, you cannot reuse or duplicate that same field within the same form. If you need to collect similar information multiple times, you must create a new field for each instance. See the Create a Custom Field article for more information.

Remove Fields From a Form
Fields can be removed from a form via the following methods:
To remove a single field, hover your mouse over the field and click the X button.
To remove multiple fields at once, click to check the box at the beginning of the field > Click the Remove Selected button.
To remove all fields at once, click the Actions button within the form > click Remove all fields.
💡Tip: Depending on the form, some contain mandatory fields marked with a red triangle
. These mandatory fields cannot be removed from a form.
Rearrange Fields on a Form
Easily reorganise your form by using the drag handle on the left side of a field. You can also click the menu on the field to select specific actions.
For a more compact layout, you can group fields side-by-side by dragging them to the left or right of another field.
💡Tip: Fields under the Extras section of available fields on the form builder cannot be arranged side-by-side on any forms.
Edit the Form Settings
The Form Settings tab is located on the form builder. See the following settings:
💡Tip: This is an exhaustive list of all available settings. The exact options that appear will vary depending on the form type you choose.
Form Title: The form name visible to Users.
Admin Label: The name Admins see within the Admin Console.
Type: The form type is unable to be changed after creation. Different form types can behave differently and be used in different parts of the platform. See the Understanding Form Types article for more information.
Publicly accessible: Check this box to generate a public link that can be accessed without logging into Rosterfy. The URL allows you to share a form via email or embed it on a website to be completed.
Allow family member creation: Check this box to allow Users to add child family members within the form while registering.
Form automatically closes?: Check this box to set a date, time and timezone for when you would like the form to close so that Users can no longer complete it.
Category: Allows you to group forms together into categories for filtering purposes. A Form Category needs to be created before you can assign it to a form. See the Create and Update a Form Category for more details.
Show confirmation page?: Check this box to add a confirmation step between pages on multi-page forms. This shows Users a brief message confirming their information was saved before they move on to the next page. Confirmation pages are only available for forms with more than one page.
Rules: Any rules applied to the form or its fields will display here. If no rules apply, it will display as [].
⚠️Warning: This is an advanced field and Rosterfy does not recommend to edit it manually.
Ask user if they need accessibility mode: Check this box to ask Users at the start of the form if they need accessibility mode enabled. Selecting this adjusts portal features such as contrast and layout, to better support visually impaired Users.
Prevent Form Resubmissions: Check this box to prevent Users from submitting this form more than once.
Update a Form
Click Forms > Lists.
Click the More Actions menu on the form you wish to update.
Click Update. The Form Builder appears:
Edit the form as desired.
Click the Save button.
Bulk Update Forms
If you wish to update multiple forms simultaneously, you can apply a Bulk Update action.
Click Forms > Lists.
Click to select specific forms you wish to include in the bulk action, or leave them unselected to apply the action to every form in the Forms List.
Click the Actions button.
Click Bulk Update Forms. The Bulk Update Forms window appears:
Click to turn off the Don't Update button to modify the following settings listed in the Edit the Form Settings section:
Form automatically closes?
Category
Show confirmation page?
Ask user if they need accessibility mode
Click the drop-down menu on the Apply To field to select one of the following options:
Forms listed: Apply the bulk action to all forms in the Forms List.
Forms selected: Apply the bulk action to selected forms.
Click the Update button. The following window appears:
Click the Yes button. The following window displays the progress of your action:
Click the Close button to exit the window.
